We are required by data protection laws to protect your personal information (data). This applies to you, carers, your relative or authorised third parties including, your GP or another service. We always seek your permission to advise or inform other people involved in your health care if necessary.
We process your data. This means collecting, recording, organising, storing, sharing or destroying data.
This information explains your legal rights when we hold your data (usually, information given by yourself to us at Open Door Therapy and Health.

Your Rights: Right of Access
Under Data Protection law you have the right to request a copy of all documents held by us. We don’t charge for this information because it is yours.

Right to Rectification
You have the right to ask us to correct any data which you believe is inaccurate or incomplete.
Your Right to Erasure
You have the right to ask that we erase any of your personal data which is no longer necessary for the purpose we originally collected it for.
Retention of your records
We hold on to your data (retain) in line with GDPR (General Data Protection Regulation).
Your right to restriction of processing
You can restrict the processing of your data if we no longer require your personal data for processing purposes.
Your right to object to processing
You have the right to object to the processing of your data.
If you request any of these things under GDPR we may ask to see personal I.D. to ensure that data is not shared with the wrong person inappropriately.

Information we collect
To help us provide a safe and professional service, we process the following types of data under Article 6.1(c) of the General Data Protection Regulation:
Your name, address, date of birth, telephone number.
We also collect classified data under Article 9.2(h) which can include health and social care data about you including, physical and mental health.
We may process data about your race, ethnicity, sexual orientation, disability or religion/spiritual/philosophical beliefs.
Where this data is collected via a survey rather than during the course of treatment, it will not be processed onwards unless processed without identifying markers, for example your name and address.
This means your data is private and protected.
We may use your personal information to:
Provide you with the important advice and support you’ve asked us for.
Provide or administer activities relating to all our services: updating you with important administrative messages, to help us identify you when you contact us and enable us to keep accurate records.
Special Category Information
We only use this information for the purposes of dealing with your enquiry, treatment provision and quality assurance monitoring.
We will not pass your details to a third party without seeking your consent in the first instance.
For adults, we keep your records for 8 years.
For under 18 years, we retain your records until you reach the age of 25.
In exceptional circumstances, where there is a risk of harm to either yourself or another person, we may share information with your doctor, health service, parent or guardian.
